Hello, if you are a young person and would like to talk and gain support from a School Nurse, you can complete this short self referral form.
Or you can ask a parent & carer to make a referral for you, but you will need to give your permission for this. If you are under 12 years old then parent & carer will need to complete the referral with you.

If School Nursing is the best service to help you, we will offer a first appointment. However, if another service may be better suited to meeting your needs, with your consent, we will arrange onward referrals to more appropriate services.
