Right Care, Right Place
Right Care, Right Place is the name of the programme of Community Transformation to support delivery of the model of care described in the SCP. The programme has 3 distinct but closely related areas of health and social care:
- Home Teams
- Care and Support at Home
- Intermediate Care and Support Living
In 2022/23, the programme has focused on the Intermediate Care and Support Living. Information packs were developed to support people to have a better understanding of the communities and the issues affecting them.
Care Home Testing Implementation Plan

Health and Care Experience Survey 2022
The Scottish Health and Care Experience (HACE) survey is carried out every two years using a postal survey sent to a random sample of people aged 17 or older and registered with a GP in Scotland. The most recent survey was sent out in November 2021 to find out about people’s experience in the previous 12 months.
COVID-19: An analysis of Care Home resident and hospital discharge data
This report describes the results of analysis of data relating to people discharged from hospital to a Care Home in Dumfries and Galloway and COVID-19. This report focuses on the first wave of the COVID-19 pandemic (01 March to 17 June 2020).

Scottish Index of Multiple Deprivation 2020 (SIMD20) Summary
This document is an overview of the Scottish Index of Multiple Deprivation published on 28 January 2020 (SIMD20) and the specific detail for Dumfries and Galloway. The SIMD20 is an area measure of deprivation, rather than one which identifies individual people. It is designed to identify the neighbourhoods and communities that have the most challenging lack of resources and opportunities across Scotland.
